When Aritani is pushed in front of a train to cover a large embezzlement, BJ buys the hospital he’s in in order to save his life.

After seeing him at Kreutzer’s hospital, BJ goes to talk to Stein, who runs a hospital in Germany. There he meets a familiar face and finds out an orphan and a pianist is going to get a replacement robotic arm. Upon learning from Pinoko that anyone could play the piano with them, he asks for an alternative. After BJ treats the patient, Stein flees to England.

Black Jack and Pinoko go to Europe to see Dr Kreutzer, an old colleague of Dr. Honma. There, he meets Suzie, a girl who thinks he’s a devil because he wears black. The hospital’s controller, Devon, wants BJ to pretend to be Kreutzer in order to buy time to pick a new director. BJ refuses, until he finds out the patient is Suzie’s mother.

BJ is given a chance to get a medical license, but he’s upset the director of the medical association. An Italian billionaire tries to have him released to treat his grandson, but fails. The Medical association send Dr White instead, but the boy dies. Furious, the billionaire has the director’s son shot. BJ is bailed out to save the day.
